1 Organizations that are USMBHA Institutional Members may enroll up to 20 staff members at this rate, per person.

2 Registration fee for Non-members includes membership to the USMBHA. If you register for this
category, please also fill out membership form.

3 Proof of full-time enrollment must be submitted with registration in order to obtain student fee.
Registration for students does not include admission to the Awards Dinner.

4 Organizations that are not USMBHA Institutional Members must enroll a minimum of 5 persons at this rate, per person.

Cancellation Reimbursement Fee

In order to cancel the registration for the 66 th Annual Meeting once it has been paid, a written notice should be sent. There will be an additional charge of $10.00 to cover the administrative costs of handling cancellation.
